| Score | Description | 1 | Death | 2 | Persistent Vegetative State: The patient exhibits no obvious cortical function. | 3 | Severe Disability: The patient depends upon others daily for physical or emotional needs. | 4 | Moderate Disability: The patient is able to function independently but has a disability i.e. ataxia. | 5 | Good Recovery: The patient is able to resume normal activities even if there are minor neurological deficits. |
